Immunization Rules

All students must have updated immunization records and a birth certificiate on file in the office.

Kindergarten-8th grade must also have the Hearing, Vision and Dental form on file.

*There is a shot that all sixth graders must have.

1) Two (2) doses of Measles Vaccine
2) Two (2) doses of Mumps Vaccine
3) One (1) dose of Rubella Vaccine
OR Laboratory proof of Immunity against Measles, Mumps and Rubella

AND

1) Two (2) does of Varicella (Chicken Pox)
OR Laboratory proof of Immunity against Varicella (Chicken Pox)
At the time your child entered school, only one (1) does of this vaccine was required.

** Contact your health care provider to receive any needed vaccinations and to obtain the Georgia Certificate of Immunization (Form 3231) **
